- Rearrange/style tpa-submit page
- Update supporting components to better match heritage
    - DeductibleBox
    - ReviewBlock
    - Alert
- Update store logic to actually handle extension
- Add/update recal modal and contact details modal
- Update titleCase to treat dashes like spaces
